Transition Services are provided to Medicaid Members who reside in a Nursing Facility and are capable of living independently.  We assist with transition to independent living through life skills training, peer mentorship, and supported employment.

As you step into a life of independence as a Medicaid member, getting familiar with your new responsibilities and your new neighborhood can be very overwhelming.  We provided an Independence Coach to work alongside of you as you navigate the world around you.  Life Skills training, peer mentorship, and supported employment services can give you the success you seek.  Click the link below to find out more about what a Life Skills Trainer and Independence Coach can do for you and how you can sign up for our services.
