CHT a PASA agency, has been a Medicaid Provider since 2018.
As we navigated the complex issues of supporting those who live
independently, we felt there was a need for more
person-centered support. As a result, we opened our
Life-Skills Training Division and our Supported Employment
Division in 2022.
Transition Services are provided to Medicaid Members who reside in a Nursing Facility and are capable of living independently. We assist with transition to independent living through life skills training, peer mentorship, and supported employment.
As you step into a life of independence as a Medicaid member, getting familiar with your new responsibilities and your new neighborhood can be very overwhelming. We provided an Independence Coach to work alongside of you as you navigate the world around you. Life Skills training, peer mentorship, and supported employment services can give you the success you seek.
