Transition Services
Transition Services are provided to Medicaid Members who reside in a Nursing Facility and are capable of living independently. We assist with transition to independent living through life skills training, peer mentorship, and supported employment.
Life Skills Training
As you step into a life of independence as a Medicaid member, getting familiar with your new responsibilities and your new neighborhood can be very overwhelming. We provided an Independence Coach to work alongside of you as you navigate the world around you. Life Skills training, peer mentorship, and supported employment services can give you the success you seek. Peer Mentorship
Have you been living a successful independent life and want to mentor those who are just starting on their journey? Reach out to our office to become a peer mentor to help others transition to independent living and we can get you started.

MEET DEBBIE JUNE...cht COLORADO pay tribute
We met Debbie June in 2019. She was our 5th client in our newly established agency. From the moment I met June I knew we would be friends. She was softly sweet and so appreciative of the job I was doing for her. My job was to help Debbie June move out of the Skilled Nursing Facility into her own apartment. There was a lot of paperwork and steps to take and she never got discouraged.
June shared with me about her struggles in life. She and I had many conversations about living independently and all of the stress that comes with being in your own apartment. She was determined to do all the right things, leave her struggles in the past and live the new life that was being presented to her. She was very excited to have her family come and visit.
What a success story June is. She stayed focused and created relationships and friendships and was always kind and grateful. I loved to visit with her and watch her face light up when she talked of her grandchildren. She would get her box of stickers and cards and with a full heart send us cards during the holidays or just to say hi.
It is because of Debbie June's commitment to live a happy and successful next chapter that we have created the Debbie June Award. We will be granting the award to one of our clients who holds the same values as June did. The commitment to self, family and happiness.

Michael was living out of a shelter before this program and now he lives in his OWN apartment! He told CHT that he is happy and truly thankful for all the support. We, at CHT, believe in second chances and Michael is living proof of them.

Lion didn't have a place to call home for some time so when he walked into his new apartment for the first time, tears streamed down his face. He is the happiest he has been in a long time and feels that he has a new start. His new start has also allowed him to reconnect with family.

Mark was homeless for nearly 10 years. He was living out of an RV for most of the time with no working water or electricity. He told CHT that without this program, he wouldn't have been able to get a home. Mark said he is so thankful for our help and we are so glad we could help him out!
