CHT is a community support agency that was create in 2018. Born from a deep understanding of the challenges faced by individuals navigating housing, healthcare, and supportive services, CHT was created to bridge the gaps in the system—with compassion, consistency, and creativity.Since our founding, we’ve helped countless individuals access safe housing, connect with resources, and develop the confidence and skills to thrive. Our work is grounded in relationships, driven by advocacy, and shaped by the resilience of the people we serve every day. At CHT, we don’t just assist—we walk alongside our clients, ensuring they are seen, heard, and supported.

Meet Corey

Corey was involved in a devastating car accident that led to a vehicle fire, leaving him with severe burns covering 60% of his body. Unable to work during his recovery, Corey lost his housing and was forced to live in a hotel while navigating one of the most difficult periods of his life.

CHT stepped in to provide stability when he needed it most—securing safe housing and walking alongside him every step of his recovery journey.

While programs like Medicaid provide critical support, they cannot meet every need. Stories like Corey’s highlight the gaps that exist—and the importance of organizations like CHT in ensuring individuals don’t fall through the cracks.

June Trujillo Resilience Award

We met Debbie June Trujillo in 2019.

She was our 5th client in our newly established agency.  From the moment I met June I knew we would be friends.  She was softly sweet and so appreciative of the job I was doing for her.  My job was to help Debbie June move out of the Skilled Nursing Facility into her own apartment.  There was a lot of paperwork and steps to take and she never got discouraged.

 

June shared with me about her struggles in life.  She and I had many conversations about living independently and all of the stress that comes with being in your own apartment.   She was determined to do all the right things, leave her struggles in the past and live the new life that was being presented to her.  She was very excited to have her family come and visit.

 

What a success story June is.  She stayed focused and created relationships and friendships and was always kind and grateful.  I loved to visit with her and watch her face light up when she talked of her grandchildren.  She would get her box of stickers and cards and with a full heart send us cards during the holidays or just to say hi.

 

It is because of Debbie June's commitment to live a happy and successful next chapter that we have created the Debbie June Award.  We will be granting the award to one of our clients who holds the same values as June did.  The commitment to self, family and happiness.
