CHT's Life Skills Training Division is dedicated to the personal growth and wellbeing of our clients.  We prioritize relationship and connection with our clients and our experienced staff focuses on setting a comprehensive coaching plan to support the client for success.


What skills are taught?

What is life skills training

 The LST service provides support for life skills that you will need when living independently.  Our trainers work with our clients' setting goals, learning new skills and organizing their life as they start their new chapter.

Skills such as:

  • community travel training: buses. light rail, Assess A Ride and taxi.
  • computer and phone use and navigation.
  • organizing and managing their household: scheduling in-home help scheduling appointments, creating menus, grocery lists etc.
  • organizing and managing their budget
  • MOST IMPORTANTLY - creating a social calendar: getting to know the neighborhood, registering for classes or hobbies, meeting new friends. 

Living with purpose and responsibility is very supportive in having a successful independent life.

The Life Skills Training is available to anyone on the following waivers:


  • EBD - elderly, blind and disabled
  • SCI - spinal cord injury
  • SLS - supportive living services

How do we request life skill training?

There are a couple ways in which you can request to work with a Life Skills Trainer.


If you have a Case Manager

Call your Case Manager and request to work with a Life Skills Trainer with CHT.


If you DO NOT have a case manager

and live in: Jefferson County 

Lakewood, Golden and Morrison and surrounding areas

Coming from a structure of dependent living and starting a new chapter can be very overwhelming.  The Independent Coach is there to guide the client through the important steps and crucial responsibilities to keep the overwhelm, anxiety and loneliness from challenging the client's success.

A task as simple as creating a budget is very important and has challenges that can often derail  a member's month spending leaving them worried and anxious.

We have found that many of our members are very excited to move out of the Nursing Facility.  They look forward to having their own schedule and not sharing a room with a roommate.  They often say they look forward to having peace and quiet. 


Yet once they are in their home, the feelings of loneliness and isolation begin to creep in and opens the door to increased anxiety and depression.


At CHT, we are aware of this psychosocial connection and have implemented tools to support connection by mitigating isolation.


The LST team focuses on getting the member connected to their community and meeting new friends.

How is this accomplished?  

                  Creating a Connection Plan

  • The Connection Plan is a document that sets a path towards getting more connected with the neighborhood, neighbors, new friends and family by discussing the interests and comfortable engagement tactics for the member.
  •           Goals for activities are set.  
  •           Activities include trip and
  •           Walks in the mountains, 
  •           Group conversations with other like-minded and shared experienced friends.  
  •           Meet ups at coffee shops or parks.  
  •           Attending a ball game.


The best part about the connection plan is that it is 100% designed and created by the member.
